Jon Lamb, MS, PT, CMTPT

Jon Lamb is a father of two children and husband to his beautiful wife Brittney.  Their family currently resides in Hoover, Alabama.  He is very active in his church as a deacon, Sunday School teacher, participating in missions, and serving on the Mission Committee. He and his wife are particularly involved in local missions as well as missions in Asia, Tanzania and Kenya. He is currently a member of the Board of Directors for the Anza Imani Rescue Foundation. Jon graduated with honors from the University of North Georgia Physical Therapy Program (Master's degree) in 1998 and received his undergraduate degree (BS) with honors from Abilene Christian University in 1995. He has been practicing within the greater Birmingham area since 1998. During his time as a therapist, he has been published in peer-reviewed journals and has been involved with ongoing research in the field of Physical Therapy. He has also lectured at local and national Physical Therapy continuing education events. He has served as an associate professor at Jefferson State Physical Therapy Assistant Program and as a Clinical Instructor for Physical Therapy Programs at UAB, South Alabama, Alabama State and other southeastern PT Programs. He has also served as the Chairman of the Advisory Committee for the Jefferson State Physical Therapy Assistant Program. Jon received his certification in dry needling through Myopain Seminars (http://myopainseminars.com/) and is a Certified Myofascial Trigger Point Therapist (CMTPT). While practicing as a Physical Therapist, Jon's focus has been on manual therapy for musculoskeletal injuries and dysfunctions, as well as post-operative rehabilitation. He has had extensive experience with sports injuries with professional, semi-professional, collegiate, high school, and middle school athletes. Jon has been providing Physical Therapy services to the McCalla - Bessemer community since 2003.

Our History

Millennium Physical Therapy opened in August of 2008 with a vision to be the provider of choice for McCalla, Bessemer, and the surrounding areas. It has been our mission, calling and blessing to be able to use our hands-on approach to restorative therapies in a manner that brings hope and healing to those we serve.
Our patients are not just a number or a diagnosis, they become part of our family.  But, don’t take it from us—ask one of your family members or friends who has been a patient at our facility to get the real story behind Millennium Physical Therapy.

Frequently Asked Questions

How long will my appointment be?

Your first appointment will generally last from 60-90 minutes.  This appointment may take longer than subsequent appointments because the therapist will perform the initial evaluation and all new patient paperwork/insurance verification will take place at this time.  Subsequent visits should last approximately 60 minutes. 

What should I wear to the clinic?

We prefer that you wear something comfortable that will not restrict your exercises. Athletic Shoes, shorts/sweat pants, and a t-shirt are ideal.

Do I need to see my doctor before going to physical therapy?

Yes, the state of Alabama requires a referral (prescription) from your doctor.

What should I bring to my first visit?

Please bring your referral (prescription) from your doctor as well as your picture ID and insurance card/s to your first visit.  If you have had any diagnostic testing, please bring the images on a computer disc, results, and other information relevant to your current condition.

What happens if I miss a scheduled appointment?

We prefer at least 24 hours advanced notice for cancellations. Your consideration allows us to assign another person to your time slot if you cannot make it.

Is physical therapy covered by insurance?

Most health insurance plans cover physical therapy. We will verify your insurance benefits at the time of your initial evaluation and can give you an approximate cost at that time. We accept all major insurance policies and are willing to work with you financially. Financial hardship plans are available for those who qualify.

Do I have a choice in where I go for my physical therapy?

As with any health care profession, whether it's dentistry, medicine, podiatry, chiropractics, etc., you have a choice in your physical therapy provider of care as well.
